Warranty
Needed adjustments or modifications will be made, at no charge, for the warranty period of three(3) months after the date of delivery. Repairs and maintenance on sockets will be made, at no charge, for the warranty period of twelve(12)months after the date of delivery. There will be a separate (non-warranty) charge for repairs, maintenance, adjustments or modifications that are made as a result of abuse or undue rough-wear as any additional adjustments which are prescribed by a physician.
Manufacturers' warranties for components will be passed on to the patient as they apply. These warranties apply to components not directly manufactured by The Center for Advanced Orthotics and Prosthetics such as: orthotic joints, prosthetic feet, knees, liners, sleeves and other components premanufactured. Manufacturer's warranties will replace Prosthetic Care warranties. A charge for labor and expenses may apply for a component being replaced under a manufacturer's warranty to cover the cost of disinstallation, reinstallation, shipping and handling.
Failure to contact the treating practitioner, or infrequent or non-use of a device does not absolve the patient /agency from the responsibility for payment. Since the device is custom fabricated and prescribed by a physician, it cannot be returned for credit on the account. Prescribed "off the shelf" items cannot be returned for credit because of hygienic concerns.
It is the patient's best interest to communicate with the practitioner on a timely basis and to allow time to resolve any problems experienced as efficiently and quickly as possible. It is the goal of The Center for Advanced Orthotics and Prosthetics to provide the patient with the best possible care.
